Jackson County Public Health Death Records

Jackson County Public Health (JACPHP) is the local office for death certificates covering deaths in Independence and the broader Jackson County area from 1980 to the present. JACPHP is located at 313 S. Liberty Street, Independence, MO 64050. Phone is (816) 404-6415. You can also reach the office by email at VitalRecords@uhkc.org. Office hours are Monday through Friday, 8:00 AM to 4:00 PM. Express pickup is available Monday through Friday from 9:00 AM to 4:00 PM for requests submitted in advance. The fee is $14 for the first certified copy and $11 for each additional copy of the same record, in line with RSMo 193.265.

Payment at JACPHP can be made by debit card, credit card, or cash. American Express is not accepted. No personal checks. In-person requests require a valid photo ID, such as a driver's license or passport. If you are submitting by mail, a Social Security card accompanied by another form of ID may be acceptable. Mail requests must include a notarized application, a self-addressed stamped envelope, and a money order or cashier's check payable to JACPHP.

Who Can Request an Independence Death Certificate

Under RSMo 193.255, access to Independence death certificates within the 50-year confidentiality window is limited to those with a direct and tangible interest in the record. Eligible requestors include a spouse, parents, children, siblings, grandparents, grandchildren, aunts, uncles, nieces, nephews, legal representatives, funeral directors, and those with documented property or estate interests. You will need to show a valid photo ID and documentation confirming your relationship or legal standing.

If you do not fall into one of these categories, you will not be able to get a certified copy from JACPHP or the Missouri Bureau of Vital Records while the record remains within the confidentiality period. Once a record passes the 50-year mark, it transfers from the Bureau to the State Archives under RSMo 193.225, at which point it becomes a public record that anyone can access without restriction.

Funeral directors acting in the course of their professional duties qualify to request death certificates under both state law and JACPHP's local procedures. An active retainer agreement or engagement letter is sufficient documentation for legal representatives and attorneys handling estate matters.

Missouri Bureau of Vital Records and Independence Deaths

The Missouri Department of Health and Senior Services Bureau of Vital Records in Jefferson City also holds Jackson County death records, including all Independence deaths from January 1, 1910, through the present. The Bureau is at 930 Wildwood Drive, Jefferson City, MO 65109. Phone is 573-751-6387. Lobby hours run Monday through Friday, 9:00 AM to 3:00 PM. Appointments are recommended for in-person visits. The Bureau covers the entire state and can issue certified copies of any Missouri death certificate, not just records from Jackson County.

Independence Death Index in the Missouri State Archives

The Missouri State Archives holds over 2.5 million digitized death certificates from 1910 through 1975. All Independence and Jackson County deaths registered during this period are searchable for free through the Archives Death Certificates portal. You can search by first name, last name, county, and year or month of death. For deaths from 1954 through 1975, the database supports searching by the name of a surviving spouse, father, or mother as well. Each digitized certificate shows the full name of the deceased, date and place of death, date and state of birth, parents' names, spouse's name, occupation, cause of death, attending physician, funeral home details, and burial location.

For deaths before 1910, the Missouri Birth and Death Records Database, Pre-1910 indexes microfilmed records from the 1883 to 1893 period and is also free to use. The FamilySearch Jackson County genealogy page lists additional resources including probate files, cemetery transcriptions, and church records that supplement the Archives database. Under RSMo 193.145, all modern Missouri death certificates are filed electronically through the MoEVR system, ensuring uniform registration of current records across all counties.

What Independence Death Records Contain

A certified Independence death certificate includes the decedent's full legal name, date and place of death, date and state of birth, sex, race, and occupation. Both parents' full names (including the mother's maiden name), the surviving spouse's name, cause of death and contributing conditions, attending physician, funeral home details, and burial location are also listed. The informant who provided data at the time of registration is named on the form.

Short form certificates are available from JACPHP. Long form certificates with extended medical certification language are available from the Missouri Bureau of Vital Records in Jefferson City. For most legal and financial purposes, both forms are equally valid. Mark the form type on your application when you submit your request to the state office if you specifically need the long form.

Records older than 50 years are open to the public through the State Archives without any proof of relationship or eligibility. Current records require documentation of your connection to the person named on the certificate.
