Box 40, Centerville, MO 63633. Phone is (573) 648-2498 and toll-free is (866) 579-3861. Hours run Monday through Thursday from 7:30 AM to 5:30 PM. The fee is $14 for the first certified death certificate copy and $11 for each additional copy of the same record. Birth certificates run $15. Details on available services are at reynoldscountyhealth.org.

The Friday closure is an important detail. Reynolds County is one of the few Missouri county health departments that is not open five days a week. If you plan to visit, go Monday through Thursday. Extended hours on those days, running until 5:30 PM, give you more time to come in after a normal workday. Still, it's worth calling ahead to confirm availability.

For in-person visits, bring a completed Application for Missouri Vital Record and a valid photo ID. A state driver's license, state ID, U.S. passport, or military ID qualifies as primary identification. Two alternate forms substitute for photo ID when needed. Accepted alternates include W-2 forms, Social Security cards, utility bills, payroll stubs, and letters from government agencies. Cash is accepted in person. Mail requests require a notarized application and payment by check or money order payable to the Reynolds County Health Center. Allow 2 to 4 weeks for mail requests to be processed, which is longer than many other Missouri county offices.

The Missouri Department of Health vital records page provides statewide context on the death registration process and the role local health centers play in issuing certified copies.

Missouri Bureau of Vital Records for Reynolds County Deaths

The Missouri Department of Health and Senior Services Bureau of Vital Records in Jefferson City holds Reynolds County death index records from January 1, 1910, to the present. The Bureau is at 930 Wildwood Drive, Jefferson City, MO 65109. Phone is 573-751-6387. Lobby hours are Monday through Friday, 9:00 AM to 3:00 PM. The fee is $14 for the first certified copy and $11 for each additional copy of the same record ordered at the same time, under RSMo 193.265. For deaths from 1910 through 1979, the state Bureau is the correct source since the local office holds records only from 1980 forward. Given the 2 to 4 week mail processing time at the local office, the Bureau or VitalChek may also be a faster option even for recent records.

The Missouri Bureau of Vital Records ordering page explains each step for requesting a certified Reynolds County death certificate by mail, with a downloadable application form and mailing address for the Jefferson City office.

Under RSMo 193.255, certified copies within the 50-year confidentiality window are issued only to those with a direct and tangible interest. Eligible parties include the spouse, parents, children, siblings, grandparents, grandchildren, and close relatives, plus legal representatives, funeral directors, and those with documented estate interests. Records more than 50 years old transfer to the State Archives under RSMo 193.225 and become public records available to anyone.

VitalChek is Missouri's authorized online vendor for death certificate orders. Ordering through VitalChek takes 3 to 5 business days and does not require a notarized application. Reynolds County Death Index in the Missouri State Archives

The Missouri State Archives holds more than 2.5 million digitized death certificates from 1910 through 1975. Reynolds County records from this period are free to search through the Archives Death Certificates portal. Search by first name, last name, county, and year or month of death. For records from 1954 through 1975, the database also supports searches by surviving spouse, father, or mother. Each certificate shows the full name of the deceased, date and place of death, birth information, both parents' names, spouse's name, occupation, cause of death, attending physician, funeral home, and burial location.

Reynolds County was organized in 1845 and sits in the rugged Ozark Highlands of southeast Missouri. It is one of the most sparsely populated counties in the state, which means the Archives record set for the county is small and easy to search through. The county's remote location meant slower population growth through the 20th century. A search by county and a ten-year date window typically surfaces results quickly. Iron mining and timber were major industries here through much of the early 20th century, and occupations on older certificates often reflect those sectors.

For deaths before 1910, the Missouri Birth and Death Records Database, Pre-1910 covers microfilmed records from roughly 1883 to 1893. Reynolds County records from the early registration period are very sparse, as was typical for remote Ozark counties before consistent statewide registration practices took hold. Probate court files at the Reynolds County Courthouse in Centerville, cemetery transcriptions, and church records from early local congregations are the main alternatives for pre-1910 deaths. Searching for obituaries in old local papers is also worth trying.

The FamilySearch Reynolds County genealogy page lists available record collections for the county including probate filings and early vital records indexes. Many are free online through FamilySearch and serve as a useful complement to the State Archives database, especially for the pre-1910 period when formal records are thin for this county.

What Reynolds County Death Records Include

A certified Reynolds County death certificate lists the decedent's full legal name, date and place of death, date and state of birth, sex, race, and occupation. Both parents are named, including the mother's maiden name. The surviving spouse is listed along with the cause of death and contributing conditions, the attending physician, the funeral home, and the burial location. The informant who supplied data at registration is also identified. Long form certificates with extended medical certification are only available through the Bureau of Vital Records in Jefferson City. If you need the long form, note that on your application.

Records in the State Archives from 1910 through 1975 carry the same core fields, though earlier forms are simpler and some entries may be incomplete. Cause-of-death language on certificates from the 1910s and 1920s can be archaic or reflect diseases that are now known by different names. The Archives medical terminology guide helps with interpretation. For Reynolds County, older occupations listed on certificates often reflect the Ozark mining and timber economy of the early 20th century.

Access to recent Reynolds County death certificates is restricted under RSMo 193.255. Only those with a direct and tangible interest may receive certified copies within the 50-year window. For deaths more than 50 years ago, records are public and free to search through the State Archives. No proof of relationship is needed for older records.

Under RSMo 193.145, all current Missouri death certificates are filed electronically through the MoEVR system. Every recent Reynolds County death is registered centrally and accessible through the state Bureau and the local health center once the record is complete, despite the county's remote location.
