Sullivan County Health Department Death Records

The Sullivan County Health Department is the local registrar for death certificates covering Sullivan County. The office is at #1 Hawthorne Drive, P.O. Box 129, Milan, MO 63556. Hours run Monday through Thursday from 8:00 AM to 4:00 PM. The department holds death records from 1980 forward and birth records from 1910 forward. The fee is $14 for the first certified copy and $11 for each additional copy of the same record requested at the same time, in line with the fee schedule under RSMo 193.265. The Milan office is closed on Fridays, so mail received after Thursday afternoon will not be processed until the following Monday. If your request is time-sensitive, visiting the office in person is the faster choice.

Missouri Bureau of Vital Records and Sullivan County Deaths

The Missouri Department of Health and Senior Services Bureau of Vital Records in Jefferson City holds Sullivan County death index records from January 1, 1910, through the present. The Bureau is at 930 Wildwood Drive, Jefferson City, MO 65109. Phone is 573-751-6387. Lobby hours run Monday through Friday from 9:00 AM to 3:00 PM. Because the Sullivan County local office holds death records only from 1980 forward, the state Bureau in Jefferson City is the right place for deaths between 1910 and 1979. Appointments are recommended for in-person visits to Jefferson City.

Under RSMo 193.255, certified copies of death certificates within the 50-year confidentiality period are available only to those with a direct and tangible interest in the record. Qualifying requestors include spouses, parents, children, siblings, grandparents, grandchildren, aunts, uncles, nieces, nephews, legal representatives, funeral directors, and those with documented property or estate interests. Under RSMo 193.225, certificates more than 50 years old are transferred to the State Archives and become public records open to anyone without proof of relationship.

Sullivan County Death Index in the Missouri State Archives

The Missouri State Archives holds more than 2.5 million digitized death certificates from 1910 through 1975. Sullivan County records from that period are freely searchable through the Archives Death Certificates portal. Search by first name, last name, county, and year or month of death. For deaths from 1954 through 1975, the database also allows searches by the name of a surviving spouse, father, or mother. Each certificate shows the full name of the deceased, date and place of death, date and state of birth, parents' names, spouse's name, occupation, cause of death, attending physician, funeral home details, and burial location.

Sullivan County was organized in 1843 in north-central Missouri. The county courthouse in Milan suffered a significant fire on June 26, 1908, which destroyed many early county records. This makes the State Archives database starting in 1910 especially important for Sullivan County research, as the 1910 death certificates represent the earliest reliable digital source of Sullivan County death records. The pre-1908 record losses are a known gap for genealogists working on Sullivan County family trees, and no complete workaround exists for records that were destroyed.

For deaths before 1910, the Missouri Birth and Death Records Database, Pre-1910 indexes microfilmed records from the 1883 to 1893 period. What Sullivan County Death Records Contain

A certified Sullivan County death certificate lists the decedent's full legal name, date and place of death, date and state of birth, sex, race, and occupation. The document also names both parents including the mother's maiden name, identifies the surviving spouse, states cause of death and contributing conditions, names the attending physician, and provides the funeral home and burial or cremation details. The informant who supplied information at the time of registration is identified on the certificate as well.

Long form certificates with extended medical certification language are available only from the Bureau of Vital Records in Jefferson City. If you need the long form for legal or insurance purposes, indicate that on your application before mailing it to the state office. Short form certified copies from the county health department are accepted for most standard legal purposes including estate administration, insurance claims, and Social Security benefit applications.

Historical records from the 1910 to 1975 period in the Archives database carry the same core fields, though earlier forms used in the first decades of registration were simpler and may show fewer details. The Archives publishes a medical terminology guide to help researchers interpret older cause-of-death language found on certificates from the early and mid-1900s.

Certified copies of Sullivan County death certificates filed within the past 50 years are restricted under RSMo 193.255. You must show a qualifying relationship or direct interest to receive a copy. For deaths more than 50 years ago, no proof of relationship is needed and records are freely searchable online through the State Archives portal at no cost.
