To request a certificate in person, bring a valid driver's license. The ACHD prefers a driver's license as the primary ID, though other photo IDs are accepted. If you have no photo ID, two alternate forms such as a Social Security card, payroll stub, utility bill, or government letter can substitute. The application form must show your relationship to the deceased, which the office uses to verify eligibility under RSMo 193.255.
The full list of eligible requestors in Missouri includes the registrant if still living, a spouse, parents, children, siblings, grandparents, grandchildren, aunts, uncles, nieces, and nephews.
