To request a death certificate in person, bring a completed Application for Missouri Vital Record and a valid photo ID. A state driver's license, state ID card, U.S. military ID, U.S. passport, school ID, or work ID all qualify as primary identification. If you do not have a photo ID, two alternate forms of identification are accepted. These can include letters from government agencies, W-2 forms, Social Security cards, court-certified adoption papers, insurance policies, Medicare or Medicaid cards, payroll stubs, cancelled checks, or utility bills.
Mail requests to the Morgan County Health Department must include a notarized application, a self-addressed stamped envelope, and a check or money order payable to the department. Notarization is only required when you submit by mail, not for in-person requests. Processing by mail at the local level is generally quicker than sending a request to the state Bureau in Jefferson City, which can take 4 to 8 weeks.
If speed matters, walking in during office hours is the fastest path. Same-day service is standard for in-person requests at most county health departments in Missouri, including Morgan County.

Morgan County Death Index in the Missouri State Archives
The Missouri State Archives holds over 2.5 million digitized death certificates covering the years 1910 through 1975. Morgan County death records from this period are free to search online through the Archives Death Certificates portal. You can search by first name, last name, county, and year or month of death. For records from 1954 through 1975, the database also lets you find entries using the name of a surviving spouse, father, or mother. That cross-reference feature helps when you know a relative's name but are not sure of the name on the actual certificate. Each digitized record includes the full name of the deceased, date and place of death, birth date and state, both parents' names, spouse's name, occupation, cause of death, attending physician, funeral home, and burial location.
Morgan County was organized in 1833, which gives it a long local history before statewide death registration began in 1910. For deaths before 1910, the Missouri Birth and Death Records Database, Pre-1910 covers microfilmed records from the 1883 to 1893 window. Additional pre-1910 sources include probate court records at the Morgan County Courthouse, cemetery records, church registers, and newspaper obituaries from the Versailles area.
The FamilySearch Morgan County genealogy page lists digitized collections and microfilm holdings relevant to Morgan County research, including court records and older vital records. FamilySearch offers free online access to many of these materials.
Under RSMo 193.145, all current Missouri death certificates are filed electronically through the MoEVR system. That means any record created in recent years flows through the state's centralized electronic process before it becomes accessible through normal channels.
What Morgan County Death Records Include
A certified Morgan County death certificate includes the decedent's full legal name, date and place of death, date and state of birth, sex, race, occupation, and both parents' full names including the mother's maiden name. The certificate also lists the name of a surviving spouse, cause of death and any contributing conditions, the name of the attending physician, funeral home details, and burial location. The informant who provided the data at the time of registration is also named on the form.
Long form certificates, which include extended medical certification language, are only available from the Bureau of Vital Records in Jefferson City. If you need the long form, mark that option on your application when you submit the request to the state office. The local health department in Versailles can only issue the standard certified copy.
Older records in the 1910 to 1975 Archives database contain the same core fields, though earlier certificates may have fewer details because the forms used in those decades were simpler. Medical terminology on older certificates sometimes needs extra interpretation. The State Archives publishes a medical terminology guide and a conditions dictionary to help researchers understand historical cause-of-death language, which can look very different from modern medical terms.
Access to recent Morgan County death certificates is restricted. Only individuals who can show a direct and tangible interest under RSMo 193.255 may receive certified copies within the 50-year window. For deaths more than 50 years ago, no proof of relationship is needed and the records are free to access online through the State Archives portal.
